The issue appears when the Sketch used for Extrude operation has two or more vertices in close proximity. Depending upon the Unit System used, it may not identify the distance between these vertices and consider them at one location. It is recommended to use smaller units like Micrometer in such cases, where you want to capture such small features in Sketch.
