For a MDM simulation, when creating the initial mesh, do we include the rigid body as a volume zone or only the wall zones composing that volume?

Whether or not to include the rigid body as a volume zone or as only the wall zones enclosing the volume may vary from problem to problem. If the specific boundary conditions are known and flow properties inside the volume (e.g temperature/ stresses) are not important to the problem, then only the wall zones can be included. If flow dynamics inside the volume are relevant, the rigid body needs to be included as a volume zone.
