Error: prism terminated prematurely

  Hi!
  Iam trying to run the prism mesh in ICEM CFD(After volume mesh) for 1 layer with 1.2 growth rate and I receive the following error below. Can anyone help me what's this error could mean?

  This is very generic error. It can be due to bad quality or wrong parameters etc. For creating prisms, first check tet mesh quality. Improve tet mesh quality. Then try only one layer of prism. Make sure you specifiy prism parameters correctly. And then split that prism layer into number of layers.
