Meshing problem in explicit dynamic

  Hey,
  
  Since i do research hip prosthesis forming in explicit dynamic, i find the problem which i cant define what the solution is. Some elements meshing soared up. When i check the quality of meshing on that elements, its still in average points( not lower than average). The contact is frictional, but when i change the contact into frictionless it still happen. My input force on die is 3000MN and prevelocity in 15 mm/s. can you help me why it could happen? i will attach the picture. i use istropic (young modulus and poisson ratio), density, and johnsoncook
