Hi,
This can be due to high velocity flow inside the domain, can you recheck the inlet conditions?
Also, try running the case using Double Precision.

Hi,
Is the operating point close to one of the speed curve limits? If so, can you try using exit corrected mass flow for the outlet?
Also what are your current boundary conditions?
