I have run 3 other cases successfully that are almost identical except for the flow rate profile at the boundary. I was able to successfully able to run 8 seconds with the CFD only, and the particles are injecting correctly. However, the simulation fails at t=8.4 seconds, even after I resubmitted it a couple of times to the supercomputer.
